This is a VERY common question I get asked in clinic, along with what sort of pillow you should use.
We can fall asleep in some interesting positions. And if you’re like me you’ve probably woken up in some unique positions with a sore neck.
Side Sleeper
Side sleepers generally need a larger pillow to fill in the space between the shoulder and the neck. If the pillow is too low, we generally see that people end up putting their arm underneath it to try and support their neck.
Everyone has a preference when it comes to softness or firmness of their pillow. Those that prefer a softer pillow will generally need a higher pillow as the head will compress the pillow slightly as you lie down.
Back Sleeper
Back sleepers can get away with a much smaller pillow.
This is due to not much space to fill in between your back and your head.
Again, the firmness is preference based.
Tummy Sleeper
Where do we start with tummy sleepers??!
Imagine having to twist your head and neck around at right angles just to breathe and then spending 6-8 hours in that position!
From an Osteopathic point of view, we prefer people not to sleep on their tummy due to the awkward position it puts the body into. With all that said, many people are tummy sleepers & have no issues at all with it.
Consider a change in your sleeping position if you’re experiencing persistent neck pain, waking up with neck pain or waking up with headaches as this may be part of the puzzle that is contributing to your complaint.

Sleeping is a deep-seeded habit and like changing any habit it can take some persistence to make a long-lasting change.
A good place to begin is start sleeping on your side with a higher pillow.
Placing a pillow between your legs/knees, & one in front of your chest to hug helps you to stop rolling over onto your tummy.
The most important thing to remember is you need to get a pillow that’s right for you and suits the way you sleep.
There is no ‘perfect’ position to be in, but instead your circumstances or your complaint may dictate what position is best for you to sleep in.
